Form for personal safety gear annual

With the new rules that allow for an annual inspection for personal safety gear, is there also a new form
that we need to fill out to get the annual for our gear?
 
As a matter of fact, when the tech person goes through your gear at the first race if they find it complete and in good order a sticker goes on the back of your racing license kind of like the helmet sticker. Easy and straightforward.
