Unfortunately my eye doctor and family doctor appointments are this week. I was lucky enough to download the 2020 form before it was taken down. I will have my doctors fill that form out and correct as needed if not acceptable.

May I ask what changes are going to be in the physical form so that I may ask my Dr. to do during the physical. This appointment took 2 months to schedule. I may impose on him to fill out the form after the fact if all that is required is done during the appointment. Thank you, John.
